This guided meditation script for dementia is written for meditation teachers, activity coordinators, care home staff, therapists and family caregivers who want a calm, simple practice to share with people living with early to moderate dementia. The language is slow, warm and concrete. Sentences are short, instructions come one at a time, and the imagery draws on familiar comforts such as sunlight, a soft blanket, a warm cup and a quiet garden, so the listener can follow along without needing to remember what came before.
You can use this script one to one at the bedside, in a favorite chair, or with a small group in a quiet room. It works well before rest, after a busy or unsettling part of the day, or as a gentle shared ritual that brings a sense of safety and connection. The script below is written to be read aloud, word for word, directly to your clients. Read it slowly, let your voice stay soft and even, and feel free to repeat any line that seems to land. This practice is offered as gentle comfort and relaxation support; it is not a treatment for dementia and is not a substitute for medical or professional care.
Let's get started.
Guided Meditation Script for Dementia:
Hello. I am right here with you. We are going to take a few quiet minutes together. There is nothing you need to do and nothing you need to remember. You can simply listen to my voice.
Let yourself sit comfortably. Feel the chair holding you. Feel how steady it is underneath you. You are safe here.
(Pause)
If it feels nice, you can let your eyes close. Or you can keep them open and look at something soft in the room. Either way is just right.
Now let’s notice the breath. Breathe in slowly through your nose… and breathe out gently. Good.
Breathe in again… and breathe out. Nice and easy. There is no wrong way to breathe.
(Pause for several breaths)
Let your hands rest in your lap. Feel how warm your hands are. You might like to rest one hand over the other. Feel the gentle weight of your hands.
Let your shoulders drop down a little. Let them soften. Like letting go of a heavy bag you have been carrying.
(Pause)
Let your face relax. Your forehead can be smooth. Your jaw can be loose. You might even feel a small smile.
Now feel your feet on the floor. Your feet are resting. The floor is solid and strong. It holds you up.
Breathe in… and breathe out. You are doing beautifully.
(Pause for several breaths)
Now I would like you to imagine a warm, soft blanket. Imagine it being laid gently over your shoulders and your lap. It is cozy and light. Your favorite color, if you like.
Feel how the blanket keeps you warm. Feel how comfortable it is to be wrapped up and cared for.
(Pause)
Now let’s imagine a lovely garden on a sunny afternoon. You are sitting on a comfortable bench. The sun is warm on your face. Not too hot. Just pleasant and kind.
Look around the garden. You might see green grass. You might see flowers in bright colors. Red, yellow, pink or white. Whatever flowers you enjoy.
(Pause)
Listen. You might hear birds singing softly in the trees. A gentle tweet, tweet. A light breeze moving the leaves.
Feel the breeze on your cheek. It is soft and fresh. Breathe it in slowly… and let it out.
Now imagine reaching out and gently touching a flower near the bench. Feel how soft the petals are. Like velvet. Smell its sweet, gentle scent. Breathe it in… and let the breath go slowly.
There is plenty of time. You can stay here in the garden for as long as you like. Everything around you is gentle and kind.
(Pause for several breaths)
Now imagine someone kind has brought you a warm cup. Maybe it is tea. Maybe it is coffee or warm milk. Whatever you like best.
Hold the warm cup in your hands. Feel the warmth spread into your fingers. Smell the lovely, familiar smell.
Take a slow sip if you like. Warm and comforting. Everything is calm here.
(Pause)
Notice how peaceful it feels to sit in the sunshine. Nothing to rush. Nowhere to go. Just you, the garden and the warm sun.
If any worry comes along, that is alright. You can let it float away like a little cloud in the sky. Watch it drift away… slowly… slowly. And the sky is blue again.
(Pause for several breaths)
Now place one hand gently on your chest. Feel your heart there. Feel your breath moving under your hand. Up… and down.
Say quietly to yourself, or just listen as I say it: I am safe. I am cared for. I am at peace.
Again: I am safe. I am cared for. I am at peace.
(Pause)
Breathe in the warm sunshine… and breathe out any tiredness. Breathe in calm… and breathe out gently.
You are loved. You are not alone. I am right here with you.
(Pause for several breaths)
Now it is time to slowly come back to the room. The garden will always be there for you whenever you want to visit.
Feel the chair under you again. Feel your feet on the floor.
Wiggle your fingers a little. Wiggle your toes.
Take one more slow breath in… and a gentle breath out.
When you are ready, you can open your eyes. Look around slowly. Notice something you can see. Maybe a color, a picture or a friendly face.
You did wonderfully. Thank you for sharing this quiet time with me.
Tips for Facilitators
Watch the listener rather than the clock. If attention drifts, shorten the garden section and move straight to the hand on the chest and the gentle return.
Adapt the images to the person. Swap the garden for a beach, a kitchen or a favorite room from their life, and swap the drink for whatever they enjoy.
Keep the setting quiet and well lit, sit at eye level, and offer a gentle hand on the arm only if the person welcomes touch.
